Required Medical Forms

Each year Winchester Thurston School requires that students have health forms on file in order to ensure that the school meets the health needs of each student, and that each student is in compliance with Allegheny County and Pennsylvania state health requirements.

Health forms are emailed to all families in May. If you have misplaced the email, or if you need additional forms, you may download any form by using the appropriate links below.

Please have the forms completed and returned to the School Nurse by August 1, 2020.

All students entering Pre-K – grade 12 (new and returning) complete, as needed:

• If medication is needed during the school day, Medication Permission Form—Completed by a parent and a physician
• If your student has any food allergies, Food Allergy Action Plan—Completed by a parent and a physician

All NEW students entering grades 1 – 12 are required to have the following health forms:

• Physician's Report —Completed by a physician
• Student Immunization Record – Signed by a physician
• Dental Health Report —Completed by a dentist

All Pre-Kindergarten students are required to have the following health forms:

• Physician's Report – Completed by physician
• Student Immunization Record – Signed by a physician

All Kindergarten students are required to have the following health forms:

• Physician's Report – Completed by physician
• Student Immunization Record – Signed by a physician
• Dental Health Report – Completed by a dentist
• Student Health History – Completed by a parent/guardian

Except for the "as needed" forms listed above, returning first graders have no required health forms.

Except for the "as needed" forms listed above, returning second graders have no required health forms.

All returning third graders are required to have the following health form:

• Dental Health Report – Completed by a dentist

Except for the "as needed" forms listed above, returning fourth graders have no required health forms.

Except for the "as needed" forms listed above, returning fifth graders have no required health forms.

All returning sixth graders are required to have the following health form:

• Physician's Report —Completed by a physician

All returning seventh graders are required to have the following health forms:

• Dental Health Report —Completed by a dentist
• Student Immunization Record (to include Meningitis Vaccine and TDAP) —Signed by a physician

Except for the "as needed" forms listed above, returning eighth graders have no required health forms.

Except for the "as needed" forms listed above, returning ninth graders have no required health forms.

Except for the "as needed" forms listed above, returning tenth graders have no required health forms.

All returning eleventh graders are required to have the following health form:

• Physician's Report —Completed by a physician

All returning twelfth graders are required to have the following health form:

• Student Immunization Record (to include second Meningitis Vaccine)—Signed by a physician
