Health Forms for Athletes

All students competing in athletics must complete and turn in all medical paperwork by the first day of practice in order to participate. For the 2010 - 2011 school year, physicals must be obtained after June 1, 2010.

All athletes are required to complete the PIAA Comprehensive Initial Pre-Participation Physical Evaluation.

Each subsequent sports season (winter, spring) will require page five of the PIAA form to be updated by a parent and turned into Ms. Leive before the start of the season.

Any students without proper medical clearance will not be permitted to attend any practices or games until such clearances are complete.
